1 | BLK | Black | A person having origins in any black racial groups of Africa. |
2 | WHI | White | A person having origins in any of the peoples of Europe, North Africa or the Middle East. |
3 | WBH | Black-Hispanic | A person having origins in any black racial groups of Africa AND is of Mexican, Puerto Rican, Cuban, Central or South American or other Spanish culture or origin. |
4 | WWH | White-Hispanic | A person having origins in any of the peoples of Europe, North Africa or the Middle East AND is of Mexican, Puerto Rican, Cuban, Central or South American or other Spanish culture or origin. |
5 | I | American Indian or Alaskan native | A person having origins in any of the original peoples of North America, and who maintains cultural identification through tribal affiliation or community recognition. |
6 | API | Asian or Pacific Islander | A person having origins in any of the original peoples of the Far East, Southeast Asia, the Indian subcontinent, or the Pacific Islands; this area includes, for example, China, India, Japan, Korea, the Philippine Islands, Samoa, etc. |
7 | U | Unknown | A person whose racial and/or ethnic origin is unknown or one who refuses to disclose his racial and/or ethnic origin. NOTE: 7 - Unknown will not be used if the reporting officer is able to observe the complainant, victim or offender to make a judgment. |
