Unknown Addresses

This is a sexual assault case. According to the nurse, the victim wants to stay partially anonymous, so they only provided the neighborhood and not a specific street or address. The report system won’t accept the case without a full address, and entering “unknown” for the street causes an error because of the ZIP code requirement.

How are we supposed to enter the address on a case report when the exact location is unknown?

You can put the address as “1 N. Unknown Ave” and then explain the actual neighborhood in the narrative section.

Another option is to use the hospital’s address on the form, and clearly note in the narrative that the real location of the incident is unknown and explain why (for example, the victim doesn’t know or doesn’t want to say).
